Metabolic Fitness Program

Provides cardiovascular disease risk assessment and consultations with a dietician and clinical exercise physiologist, followed by weekly group meetings by Zoom to teach skills related to nutrition, exercise, stress management, and strategies for behavior change to support positive lifestyle changes.

Duration

Two options: 12 and 24 weeks.

Eligibility

Metabolic conditions, including diabetes or metabolic syndrome, without significant physical injuries or limitations. Referral by a clinician is required.

Cost

$205 for 12 weeks and $377 for 24, plus any charges not covered by insurance, such as co-pays, deductibles, or percentages.


Diabetes Prevention Program

Offered by the National Kidney Foundation of Michigan and other community-based organizations, the program helps you learn how to eat healthier, lose weight, and incorporate more physical activity into your life. The goal is to help people prevent type 2 diabetes. One-hour small group sessions available in-person or online. Weekly sessions for the first 16 weeks, followed by monthly sessions.

Duration

1 year.

Eligibility

Prediabetes, history of gestational diabetes, or at risk for type 2 diabetes based on self-assessment.

Any Michigan Medicine clinician may place a referral to Diabetes Prevention Programs offered by the National Kidney Foundation of Michigan, but a referral is not required.

Cost

Prices vary by providing organization. May be no cost, covered by your insurance, or scholarships and payment plans may be available.

Lifestyle and behavior-change programs

These programs combine coaching, education, and tracking tools to help build sustainable eating, activity, and habit changes.

Please note: Some of these programs may offer treatment with weight-loss medications. It is recommended that you discuss the use of weight-loss medication with your primary care or weight management clinician before starting treatment. Some programs may offer the option of being prescribed compounded GLP-1s. These compounded medications are not FDA-approved, and we caution against their use.
